(4) Definitions In this subsection: (A) Services included in physicians’ services The term “physicians’ services” includes other items and services (such as clinical diagnostic laboratory tests and radiology services), specified by the Secretary, that are commonly performed or furnished by a physician or in a physician’s office, but does not include services furnished to a Medicare+ÐChoice plan enrollee. (B) Medicare+Choice plan enrollee The term “Medicare+Choice plan enrollee” means, with respect to a fiscal year, an individual enrolled under this part who has elected to receive benefits under this subchapter for the fiscal year through a Medicare+Choice plan offered under part C, and also includes an individual who is receiving benefits under this part through enrollment with an eligible organization with a risk-sharing contract under section 1395mm of this title . (C) Applicable period The term “applicable period” means— (i) a fiscal year, in the case of fiscal year 1998, fiscal year 1999, and fiscal year 2000; or (ii) a calendar year with respect to a year beginning with 2000; as the case may be.
